Strengthening Bridges: A Navigation Plan for survivors of breast and gynecological cancers in the Georgia Cancer Center for Excellence (GCCE) at Grady Health System.

• Implement a Survivorship Patient Navigation Plan (SPNP) that incorporates Journey Forward material and direct access to psychosocial and support services for survivors and their families;
• Define how the information in the survivorship care plan will be obtained, who will complete the survivorship care plan, and who will communicate the survivorship care plan to the survivor;
• Pilot the SPNP at the GCCE with breast and gynecological cancer survivors
• Evaluate the process of the survivorship care plan completion from the providers’ perspective and the understandability and utility from the survivors’ perspective;
• Conduct a monthly education and support group for survivors and their family members.
• Disseminate knowledge gained from this intervention, including suggesting future outcome measures and studies.
